April 13th and 14th were two inspiring and intensive days in Pavia for the official WISH kick-off.

Hosted at IRCCS Maugeri (project coordinator) the meeting brought together a strong multidisciplinary consortium of clinicians, researchers, technologists, and innovation experts, all aligned around a shared ambition: to rethink how follow-up care is delivered for women with early breast cancer.

The discussions over these two days confirmed both the complexity and the transformative potential of WISH.

At its core, the project moves beyond hospital-centric models and explores a distributed, patient-centred follow-up approach, where:

• primary care, oncology teams, and patients are connected in a shared ecosystem
• digital tools (including the WISH app) enable continuous monitoring and timely intervention
• AI-driven analytics support better decision-making and long-term health outcomes

The agenda reflected our ambition: from clinical rationale and study design, to co-design methodologies, AI-supported app development, and implementation through a multicentric randomised trial in Italy and Spain.

What stands out most:

✔ a clear focus on health equity, especially for rural and underserved populations
✔ a robust integration of clinical research, digital innovation, and implementation pathways

WISH is not just about testing a new follow-up model — it is about reconfiguring care pathways to be more accessible, responsive, and sustainable.
